Adult First Visit (Ages 15 and over)

BEFORE THE FIRST VISIT

We will request you to fill out our patient forms located in the “Patients – Forms” center on our website.  The paperwork provides us with general information about yourself.  When calling to schedule your appointment, a team member will ask you to complete these forms before your first visit to save you time in the office.  Please bring your insurance card(s) and photo ID with you.

CONSULTATION

Upon entering our office, our team will welcome you as a member of our practice.  You will begin a consultation with one of the doctors where your detailed health history will be discussed.  During this consultation the doctor will review your goals for care and how our office will be a guide for you to meet your goals. Please ask any and all questions you have.  We’re here to serve you!

EXAMINATION

After your consultation, the doctor will perform a complete chiropractic examination.  Initial tests and assessments will include orthopedic and neurological tests, range of motion, and postural exam.  Your specific condition may require us to recommend x-rays to get a full evaluation.  Because we do not recommend x-rays for every patient, these are not performed in our office and you will be given a referral for an imaging center convenient for you.  If you do not need imaging before we adjust you, relief care is offered on the first day and at-home instructions are given.

REPORT OF FINDINGS

The doctors’ top priority is detecting nervous system interference and subluxation.  Once all the examinations and information have been reviewed, the doctor will talk with you and you will be told what is wrong, if we can help you, and how long it may take.  If the doctor believes that chiropractic care cannot help your problem, they will recommend an alternate course of action.

CHARGES AND FEES

Our office does not offer generalized treatment plans or packages.  Each patient is seen and treated as an individual and your care will reflect that. 

Insurance: We accept most insurance plans and have a business office with a staff member always willing to answer questions and help you as much as possible. 

For many reasons, some people do not wish to submit their visits to insurance.  In this case, we charge a standard rate for an office visit.  The first visit, which includes a full exam as well as an adjustment, is $140.  Subsequent office visits are $55.  These costs include any therapies needed and performed.

Our doctors provide RockTape taping and this is done at $10 per treatment, if desired/needed.
